Community Hospital of the Monterey Peninsula recognizes that each person who seeks care for themselves or a loved one has specific rights related to that care. All hospital personnel will honor and observe these rights.
We further recognize that all healthcare must be carried out in a manner that respects your personal dignity, values, and beliefs, and your right to make decisions regarding your medical care, up to and including discontinuing treatment, to the extent permitted by law.
We are committed to serving the physical, psychological, social, emotional, and spiritual needs of each patient and their family. We will treat you, your family, and those responsible for you with consideration, compassion, dignity, and respect.
As a Community Hospital patient, you may exercise the following rights regardless of age, gender, economic status, race, color, national origin, religion, ethnicity, disability, educational background, marital status, sexual orientation, or source of payment for care. These rights also apply to the person who has legal responsibility for decisions concerning your care if you are unable to make decisions yourself.
- Reasonable access to care and reasonable response to requests made of your healthcare provider
- Information in a language or method you can understand
- Information regarding your rights upon admission to the hospital and prior to discontinuing your care
- Sufficient information about your illness, any proposed treatment or procedure, and the prospects for recovery so you can actively participate in making decisions about medical care and give your informed consent to receive that care
- Information about advance directives. The hospital can provide an advance healthcare directive kit (including a power of attorney for healthcare form) for you to complete. If you do not have an advance directive, you may express your healthcare wishes to your physician, nurse, or other healthcare provider
- Have your wishes, as expressed in an advance directive, honored
- Notify family members/representatives and your physician promptly of your admission to the hospital if you choose to do so
- Care that provides:
- Comfort measures, pain assessment, and effective pain management; support of grief and bereavement; and treatment of any temporary conditions which could be expected to resolve even if you choose to forego other treatments which are no longer beneficial to your condition
- Protection from all forms of abuse and harassment
- Freedom from the use of restraints except only when medically necessary. If restraints must be used, you can expect that the least-restrictive restraint needed to keep you and/or others safe will be applied. Restraints will never be used as a means of coercion, discipline, convenience, or retaliation by staff
- Know the name of the physician who has primary responsibility for coordinating your care and the names and professional relationships of other physicians and non-physicians who will see you
- Confidentiality and privacy at all times related to all aspects of your care and your medical record
- Refuse treatment and leave the hospital even against the advice of physicians
- Safe, quality care based on medical community standards and necessity
- Information concerning continuing healthcare needs following discharge from the hospital
- Reasonable continuity of your care following your discharge, advance knowledge of those who will provide your care, and information about the time and location of appointments made
- Be advised if the hospital or your physician plans to engage in or perform any experimentation affecting your care; refuse to participate in such research; and receive full disclosure of risks and benefits so you can give informed consent if you decide to participate
- Review and receive explanation of your bill, regardless of payment source
- Decide who will visit you or choose not to have visitors, following hospital visitor guidelines. Your wishes will be considered even if you cannot make a decision
- An explanation for the presence of any individual involved in your care
- Address your ethical concerns with your healthcare team members and representatives of the hospital's administration. Concerns may include conflict resolution, withholding resuscitation, withdrawal of life-sustaining treatments, and participation in investigational studies or clinical trials
- Access to your medical record within a reasonable time frame
- Access to child or adult protective services
- Have a complaint promptly addressed. There is always an administrator available to assist you

- Participation in your plan of care
We will ask you to provide accurate and current information, follow the treatment planned for you unless you disagree with it, discuss pain relief options with your doctors and nurses to develop a pain management plan, communicate your concerns, and be ready to leave when discharged.
- Financial obligations
You have a responsibility to give accurate financial information, have your health coverage cards/information available, pay your bill promptly, and apply for Sponsored Care (link) if you need financial assistance.
- Respecting hospital property
Respect for hospital property makes it available for the next person and helps control costs for everyone.
- Appropriate conduct
You have a responsibility to be considerate and respectful of others, including patients and hospital personnel. Too many visitors, loud conversations, noisy or late television programs, and other disruptive behavior can be disturbing to
These are your rights and responsibilities. Community Hospital's policies and procedures ensure that every patient and family carefully and freely exercises both. If you have any concerns, please share them immediately with your doctor, nurse, or an administrator.
