Stroke, Wellness and Primary Care Initiatives

Primary Stroke CenterPrimary Stroke Center

Community Hospital seeks accreditation from the Joint Commission as a certified Primary Stroke Center. The Joint Commission specifies guidelines to ensure the right team is trained and in place to promptly react when a stroke victim arrives in the emergency department.

New treatments may prevent death and disability, but only if they are given within the first three hours after initial symptoms of a stroke occur. Funding for this initiative will allow Community Hospital to recruit a director of neurology and fund necessary data collection, training, and public awareness.

Primary Care Initiative Peninsula Primary Care

The Primary Care Initiative is a fundamental component of the hospital's strategic plan and mission to provide quality healthcare and address the critical shortage of primary care physicians. This innovative solution will tackle a recruitment and retention challenge for primary care physicians and provide easier access for patients.

By developing physician practices, such as our planned centers in Carmel and Marina, Community Hospital will promote a more integrated relationship between physicians and patients. Funding will allow Community Hospital Foundation to actively recruit and provide key incentives to primary care physicians to practice in the community.

Wellness InitiativeWellness Initiative

The Wellness Initiative speaks to the need to intervene earlier along the continuum of health and disease.

Healthcare should begin with prevention and continue with an integrated approach to managing chronic illnesses, treating ongoing healthcare needs, and addressing life-threatening, but avoidable, diseases. Under the guidance of Community Hospital, the Wellness Initiative will offer community members a continuous strategy to manage health conditions, devise diet and exercise plans, and support emotional and physical needs.

Funding will enable Community Hospital to hire, train, and certify staff, offer educational opportunities to the community, and construct a state-of-the-art integrated medical wellness facility.
