COVID-19 vaccine appointments

To sign up for a vaccine from Montage Health, you must go through the state's website, MyTurn.

Go to MyTurn to confirm that you are eligible. If you are eligible, you will see a list of local sites offering appointments, including ours: Community Hospital of the Monterey Peninsula and Montage Wellness Center in Marina.

If you choose one of our sites, and we have available slots, please make your appointment. Once complete, you will receive a confirmation email from Montage Health.

check boxStep 3 - Go to your appointment

At your appointment, you must provide government-issued identification showing full name and date of birth. Accepted IDs include:

  • Driver's license
  • California ID card or REAL ID card (from the DMV)
  • Military ID
  • Passport

Notes for minors who are eligible

  • Pfizer is the only vaccine approved for use for minors. They must be at least 16 years of age
  • Minors must be accompanied to the appointment by a parent or legal guardian
  • For the minor's safety, please bring proof that they are at least 16, such as a birth certificate, passport, or other document that shows date of birth

syringe iconStep 4 - Make an appointment for 2nd dose

If you receive a Moderna or Pfizer vaccine, you will need a second dose for the vaccine to be fully effective. You will be able to make your second-dose appointment at the clinic, after you receive your first dose. This second-dose appointment must be made in MyChart, our free patient portal.
