COVID-19 vaccine appointments
To sign up for a vaccine from Montage Health, you must go through the state's website, MyTurn.
Step 1- Are you eligible?
Go to MyTurn to confirm that you are eligible. If you are eligible, you will see a list of local sites offering appointments, including ours: Community Hospital of the Monterey Peninsula and Montage Wellness Center in Marina.

Step 2 - Make an appointment
If you choose one of our sites, and we have available slots, please make your appointment. Once complete, you will receive a confirmation email from Montage Health.
Step 3 - Go to your appointment
At your appointment, you must provide government-issued identification showing full name and date of birth. Accepted IDs include:
- Driver's license
- California ID card or REAL ID card (from the DMV)
- Military ID
Notes for minors who are eligible
- Pfizer is the only vaccine approved for use for minors. They must be at least 16 years of age
- Minors must be accompanied to the appointment by a parent or legal guardian
- For the minor's safety, please bring proof that they are at least 16, such as a birth certificate, passport, or other document that shows date of birth
Step 4 - Make an appointment for 2nd dose
If you receive a Moderna or Pfizer vaccine, you will need a second dose for the vaccine to be fully effective. You will be able to make your second-dose appointment at the clinic, after you receive your first dose. This second-dose appointment must be made in MyChart, our free patient portal. (If you don’t have a MyChart account, you can sign up at www.montagehealth.org/mychart.)
