Planning for and talking about what we want when it comes to serious illness and end-of-life care is difficult, but delaying or avoiding this important planning leads to increased suffering and stress for ourselves and our loved ones.
Plan ahead by attending a free workshop to:
- Select the best document to record your wishes
- Review any advance planning documents you have to be certain they are current
- Receive expert coaching to determine who will be your spokesperson and to clarify your wishes
- Submit your documents into your Community Hospital medical record

Physician Orders for Life-Sustaining Treatment (POLST) is a form that gives seriously-ill patients more control over their end-of-life care, including medical treatment, extraordinary measures (such as a ventilator or feeding tube) and CPR. The Coalition for Compassionate Care of California has created and compiled a variety of resources to help educate patients, caregivers, and healthcare professionals on the issues – both medical and deeply personal – surrounding POLST.
