Chaplain Services Volunteer Form

Please use this online form to provide the Chaplains' office with contact information and details regarding your interest in working with our chaplain services. Applicants must represent a specific faith tradition and local congregation, be educated, licensed and/or ordained for ministry within their tradition and have appropriate experience or education in pastoral care and/or tradition-specific ministries suitable for use within a health care environment. The Chaplain Coordinator will review your information and contact you within a few days.

VOLUNTEER CHAPLAINS are required to have either CPE units or like experience within a clinical health care setting. Volunteer chaplains visit any patient identified as needing pastoral support. Requirements include: proof of pastoral credentials, current good standing in a local faith community and demonstrated clinical chaplain experience in a hospital or like setting.

VOLUNTEER LAY LEADERS (Eucharistic Ministers, Stephen Ministers, Missionaries, etc.) AND TRADITION SPECIFIC CHAPLAINS are required to provide proof of pastoral credentials (training and supervision for your ministry) and a letter of recommendation from the leader of your local faith community clarifying your training and yearly confirmation of your appointment to serve in this capacity. These volunteers visit patients who have requested tradition-specific pastoral, ritual or sacramental support.
